Immunization Registry

Doctors of Internal Medicine participates with Immtrac2, the state of Texas Immunization Registry offered by the Texas Department of State Health Services (DSHS).  The registry is secure and confidential, and safely consolidates and stores immunization records from multiple sources in one centralized system.  Texas law requires written consent by individuals to participate in the registry and can only be accessed by authorized organizations.  Authorized organizations include health care providers, schools, and public health departments. The registry is part of the state’s initiative to increase vaccine coverage across Texas.

Consent forms for both adults and children can be obtained from Texas Health and Human Service or on the web and are available in both English and Spanish.
